Findings from literature support the efficacy and safety of resective surgery in well selected patients. Nevertheless epilepsy surgery remains underused; recent studies show a low rate of surgical procedures over time. This stagnation is related to numerous barriers including fears of complications and inadequate knowledge of surgical perspective. Some strategies should be developed to increase access to surgery and to improve communication about the effectiveness of this potentially life-changing approach.
